Radiology Referral Form
To refer a patient for an outpatient ultrasound at Memphis Veterinary Specialists & Emergency, please submit the form below or download and fax to (901) 624-9014. This service is intended for patients who need an abdominal or cardiac ultrasound to go along with the work up you have already performed.
Important Notice
Due to the volume of referrals we receive, please allow up to 2 business days for our team to process your referral. A member of our staff will contact you to schedule an appointment.
If your patient's condition requires emergency intervention, please call our Emergency Department at (901) 624-9002 to speak with the emergency DVM on staff.
