Surgery Referral Form
To refer a patient for surgery at Memphis Veterinary Specialists & Emergency, please submit the form below or download and fax to (901) 624-9014.
Please do not submit a referral for emergency procedures such as hemoabdomen, or non-ambulatory/ IVDD.
Please call the hospital and speak with the ER doctor.
Important Notice
Due to the volume of referrals we receive, please allow up to 2 business days for our team to process your referral. A member of our staff will contact you to schedule an appointment.
If your patient's condition requires emergency intervention, please call our Emergency Department at (901) 624-9002 to speak with the emergency DVM on staff.
